Indications for Intrauterine Insemination in Ukraine

In Ukraine, intrauterine insemination (IUI) is prescribed for both couples with infertility and physiologically healthy women who use donor sperm for conception.

 The main indications for intrauterine insemination are as follows:

  • Cervical factor infertility in women: narrowing or scarring of the cervical tissues.
  • Vaginismus: spasms of the vaginal and pelvic floor muscles.
  • Presence of antisperm antibodies (immune infertility).
  • Ejaculation disorders in men.
  • Slightly reduced sperm concentration in men or minor changes associated with their motility.
  • Infertility of unclear etiology (after examination, both partners are healthy, but conception does not occur).

ICSI Clinic conducts two types of intrauterine insemination (IUI):

Natural cycle:
Natural cycle:
This procedure does not involve any medication intervention and is as close to natural conception as possible.
Stimulated cycle:
Stimulated cycle:
This involves hormonal therapy to stimulate the simultaneous maturation of multiple eggs, thereby increasing the chance of fertilization after IUI.

Preparation for IUI begins with a comprehensive examination of the couple. The doctor determines their program during the first consultation before the menstrual cycle. During the consultation, it is necessary to bring medical records and current results of previous examinations.

The comprehensive examination for women includes the following tests:

  •   Complete blood count (valid for 1 month);
  •   Coagulogram (valid  for 1 month);
  •   Vaginal swab for flora (valid for 1 month);
  •   Liquid-based Pap test (valid  for 1 year);
  •   Blood tests for female sex hormones FSH, LH, prolactin, E2, AMH, progesterone (valid  for 6 months);
  •   Blood tests for thyroid hormones TSH, free T4, T3 (valid  for 6 months);
  •   Blood tests for antibodies to cytomegalovirus infection, herpes simplex virus, toxoplasmosis, rubella (if no infection is present, valid  for 6 months);
  •   Pelvic ultrasound, breast glands, and thyroid gland.

The following examinations are required for men:

  Semen analysis;

  Chest X-ray (valid for 1 year).

In addition to gender-specific tests, there are several analyses that both partners need to undergo. These include blood tests for:

  Blood type and Rh factor;

  Sexually transmitted infections (results valid for 3 months);

  Determination of karyotype.

Consultation with a family doctor or therapist may also be necessary, and in cases of chronic illnesses, specialized doctors may be consulted, and for men, a consultation with a urologist.

 Before IUI, both partners should refrain from:

  Smoking and alcohol consumption;

  Sexual intercourse for 2-3 days;

  Men should also avoid hot baths, sauna, or steam room visits for several days before the procedure.

FAQ

What is the difference between IUI and IVF procedures?
During IUI, sperm is injected directly into the uterine cavity, and fertilisation takes place in the woman's body. During IVF, the sperm and egg are combined outside the woman's body. In addition to fertilisation itself, IVF involves follicle puncture to obtain an oocyte and embryo transfer; these manipulations are not performed during IUI.
What is the probability of pregnancy after IUI?
The probability of pregnancy after 1 procedure is 10-30%.
Is it possible to choose the sex of the child with intrauterine insemination?
No, it is illegal and unethical to choose the sex of a child, but some assisted reproductive technologies allow you to determine the sex. IUI does not provide this option.
What are the possible causes of intrauterine insemination procedure failures?
The procedure may fail if the quality of the oocytes and/or sperm is poor, if the doctor's recommendations are not followed at the preparation stage or after the procedure, if there are certain gynaecological diseases that require treatment (pathologies of the fallopian tubes, endometrium) and infertility factors that are indications for IVF.
